Common use
Triumeq is a fixed-dose combination antiretroviral tablet prescribed for the treatment of HIV-1 infection in adults and pediatric patients weighing at least 40 kg. When you buy Triumeq online, you receive a single-tablet regimen containing three powerful active ingredients: abacavir (600 mg), dolutegravir (50 mg), and lamivudine (300 mg). Abacavir and lamivudine are nucleoside reverse transcriptase inhibitors (NRTIs) that block the viral enzyme reverse transcriptase, preventing HIV from converting its RNA into DNA. Dolutegravir is a second-generation integrase strand transfer inhibitor (INSTI) that prevents the HIV integrase enzyme from inserting viral DNA into the host cell genome. Together, these three drugs target two different stages of the HIV replication cycle, providing potent and sustained virological suppression, robust immune recovery through CD4 cell count restoration, and a high barrier to resistance development. Triumeq is recommended as a preferred first-line regimen by major international HIV treatment guidelines.
Dosage and direction
Triumeq combination antiretroviral tablets are taken orally, one tablet once daily with or without food. Swallow the tablet whole with water; do not crush, break, or chew. Consistent daily dosing at approximately the same time each day is essential for maintaining therapeutic drug levels and achieving optimal viral suppression. Dosage adjustments or switching to individual component drugs may be necessary in patients with renal impairment (creatinine clearance below 50 mL/min) or mild hepatic impairment. Triumeq should not be used in patients with severe hepatic impairment. Regular monitoring of HIV viral load, CD4 cell count, liver function, kidney function, and HLA-B*5701 status (prior to initiation) is recommended throughout treatment.
Precautions
Before starting Triumeq HIV therapy, all patients must be screened for the HLA-B*5701 allele, as carriers have a significantly increased risk of developing a potentially life-threatening abacavir hypersensitivity reaction (HSR). Patients who test positive for HLA-B*5701 must never receive abacavir-containing medications. Inform your doctor about any history of liver disease (including hepatitis B or C co-infection), kidney problems, cardiovascular risk factors, or a history of lactic acidosis with hepatic steatosis. Do not stop Triumeq therapy abruptly without medical supervision, especially in patients with hepatitis B co-infection, as sudden discontinuation may trigger severe hepatitis B flare-ups with rapid viral rebound and hepatic decompensation. Patients with pre-existing liver disease are at increased risk for hepatic adverse events. Monitor for symptoms of immune reconstitution inflammatory syndrome (IRIS), which may occur shortly after initiating effective antiretroviral therapy.
Contraindications
Do not use Triumeq if you have a known allergy to abacavir, dolutegravir, lamivudine, or any component of the tablet. Triumeq is strictly contraindicated in patients who are HLA-B*5701 positive or who have experienced a prior abacavir hypersensitivity reaction, regardless of HLA-B*5701 status. Do not co-administer with dofetilide, as dolutegravir increases dofetilide plasma concentrations, potentially causing life-threatening cardiac arrhythmias. Triumeq should not be used in patients with severe hepatic impairment or in combination with other abacavir- or lamivudine-containing products.
Possible side effect
Common side effects of Triumeq include headache, nausea, diarrhea, fatigue, insomnia, dizziness, and nasal congestion. Dolutegravir may cause elevated creatinine levels without actual kidney damage (due to inhibition of tubular secretion). These effects are generally manageable and often improve with continued therapy. Serious adverse effects requiring immediate medical attention include abacavir hypersensitivity reaction (fever, rash, gastrointestinal symptoms, respiratory symptoms, and constitutional symptoms occurring together), hepatotoxicity with or without rash, lactic acidosis with hepatic steatosis, immune reconstitution inflammatory syndrome (IRIS), and severe skin reactions. Any multi-organ reaction occurring within the first 6 weeks of treatment should be evaluated as potential abacavir HSR and the drug must be permanently discontinued if confirmed.
Drug interaction
Triumeq may interact with several medication classes. Dofetilide is contraindicated due to increased plasma levels. Certain anticonvulsants (carbamazepine, oxcarbazepine, phenytoin, phenobarbital) and rifampin are strong enzyme inducers that reduce dolutegravir levels, requiring additional dolutegravir dosing. Metformin levels may increase when co-administered with dolutegravir, requiring metformin dose adjustment. Polyvalent cation-containing products (aluminum, magnesium, calcium, iron supplements, antacids) can significantly reduce dolutegravir absorption and should be taken at least 2 hours after or 6 hours before Triumeq. Inform your doctor about all prescription medications, over-the-counter products, herbal remedies, and supplements you are taking to ensure safe therapy.
Missed dose
If you miss a dose of Triumeq, take it as soon as possible unless it is near the time for the next scheduled dose. In that case, skip the missed dose and resume your regular once-daily schedule. Do not take two tablets at the same time to compensate. Consistent daily adherence is critical for maintaining viral suppression and preventing the development of drug resistance.
Overdose
Triumeq overdose may cause severe nausea, vomiting, dizziness, and an intensification of known side effects. There is limited experience with overdose of the combination components. Dolutegravir is highly bound to plasma proteins and is unlikely to be significantly removed by dialysis. Seek immediate emergency medical attention if overdose is suspected and provide supportive care as clinically indicated.
Storage
Store Triumeq at controlled room temperature between 20 and 25 degrees Celsius (68-77 degrees Fahrenheit), protected from moisture, excessive heat, and direct light. Keep tablets in the original bottle with the desiccant inside and the cap tightly closed. Store out of reach of children and pets. Do not use after the expiration date.
