Common use
Nexavar (sorafenib tosylate) is a targeted anticancer therapy prescribed for the treatment of advanced hepatocellular carcinoma (HCC — primary liver cancer), advanced renal cell carcinoma (RCC — kidney cancer), and locally recurrent or metastatic, progressive, differentiated thyroid carcinoma (DTC) that is refractory to radioactive iodine treatment. Sorafenib is a multikinase inhibitor that works by simultaneously blocking multiple enzymes (kinases) involved in tumor cell proliferation, survival, and angiogenesis. Specifically, it targets RAF kinases (C-RAF, B-RAF), VEGFR-1, VEGFR-2, VEGFR-3, PDGFR-beta, KIT, FLT-3, and RET — cutting off the signaling pathways that cancer cells use to grow, divide, and recruit new blood vessel networks for nutrient supply. By attacking tumors on multiple fronts, Nexavar slows disease progression and extends survival in patients with these difficult-to-treat cancers. Dosage and direction
Nexavar is taken orally as two 200 mg tablets twice daily (total daily dose of 800 mg), approximately 12 hours apart. Tablets should be taken on an empty stomach (at least 1 hour before or 2 hours after eating) or with a moderate-fat meal — high-fat meals reduce absorption and should be avoided. Swallow tablets whole with a full glass of water; do not crush, chew, or break them. Dosage adjustments, temporary dose reductions to 400 mg daily, or treatment interruptions may be necessary to manage adverse effects, particularly hand-foot skin reaction, diarrhea, or hypertension. Treatment is continued as long as clinical benefit is observed or until unacceptable toxicity develops. Regular monitoring of blood pressure, liver function (ALT, AST, bilirubin), thyroid function, complete blood counts, electrolytes, and tumor response (imaging) is essential throughout therapy.
Precautions
Before starting Nexavar, inform your oncologist if you have liver disease or hepatic impairment (including Child-Pugh classification), kidney disease, heart problems (heart failure, coronary artery disease, QT prolongation), uncontrolled hypertension, bleeding disorders or current use of anticoagulants, a history of gastrointestinal perforation, wound healing problems, or planned surgical procedures. Sorafenib should be discontinued at least 10 days before elective surgery, as it impairs wound healing. Monitor carefully for hypertension — blood pressure should be checked regularly during the first 6 weeks and periodically thereafter. Hand-foot skin reaction (palmar-plantar erythrodysesthesia) is common and may require dose modification; use emollients, avoid hot water, and wear comfortable shoes. Report any skin changes, severe diarrhea, unexplained weight loss, chest pain, unusual bleeding, or sudden severe abdominal pain immediately. Women of childbearing potential must use effective contraception during treatment and for at least 6 months after the last dose.
Contraindications
Do not use Nexavar if you are allergic to sorafenib or any component of the tablet formulation. Nexavar is contraindicated during pregnancy and breastfeeding due to its teratogenic potential and risk of fetal harm. The combination of sorafenib with carboplatin and paclitaxel is contraindicated in patients with squamous cell lung cancer, as clinical trials showed increased mortality in this population. Use with extreme caution in patients with severe hepatic impairment (Child-Pugh C), as safety and efficacy have not been fully established.
Possible side effect
Common side effects of Nexavar include diarrhea (often dose-limiting), fatigue, skin rash, hand-foot skin reaction (redness, swelling, pain, blistering on palms and soles), alopecia (hair thinning), nausea, vomiting, loss of appetite, weight loss, hypertension, and abdominal pain. Serious side effects requiring immediate medical attention include severe hepatotoxicity (jaundice, liver failure), hemorrhage (gastrointestinal bleeding, intracranial hemorrhage, pulmonary hemorrhage), cardiac events (myocardial ischemia, infarction, QT prolongation), gastrointestinal perforation (sudden severe abdominal pain), drug-induced hepatitis, severe dermatological reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), hypertensive crisis, and impaired wound healing. Seek emergency medical care if you experience signs of serious bleeding, liver failure, cardiac symptoms, or severe skin reactions.
Drug interaction
Nexavar may interact with anticoagulants (warfarin — increased bleeding risk and altered INR), CYP3A4 inducers (rifampicin, phenytoin, carbamazepine, dexamethasone, St. John's Wort — which decrease sorafenib levels), CYP3A4 inhibitors (ketoconazole), UGT1A1 and UGT1A9 substrates (irinotecan — increased toxicity), neomycin (decreases sorafenib bioavailability), other anticancer agents (doxorubicin, docetaxel — altered pharmacokinetics), fluorouracil, and certain antibiotics or antifungals. Inform your oncologist of all prescription drugs, over-the-counter medications, vitamins, and supplements you are taking to avoid dangerous interactions.
Missed dose
If a dose is missed, take it as soon as you remember unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume your regular twice-daily schedule. Do not take a double dose to compensate. Contact your oncology team if you miss multiple consecutive doses.
Overdose
Overdose of sorafenib may cause severe diarrhea, skin reactions, bleeding, liver toxicity, or cardiovascular complications including dangerous hypertension and cardiac events. Seek immediate emergency medical attention if overdose is suspected. There is no specific antidote for sorafenib. Treatment is supportive and symptom-based, with close monitoring of vital signs, organ function, and electrolytes.
Storage
Store Nexavar at controlled room temperature between 68°F and 77°F (20°C to 25°C), away from moisture, heat, and direct light. Keep tablets in the original container with the lid tightly closed. As a cytotoxic medication, Nexavar should be stored securely out of the reach of children, pets, and individuals for whom it is not prescribed. Handle with care and follow institutional or local guidelines for safe handling and disposal of anticancer medications.
