Isordil (isosorbide dinitrate) is a vasodilator belonging to the nitrate medication class, specifically prescribed for the treatment and prevention of angina pectoris — the chest pain caused by insufficient blood flow to the heart through the coronary arteries. Isordil is frequently used as a prophylactic medication before physical activities that commonly trigger angina, including exercise, sexual activity, and other exertion. Contraindications

Isordil is contraindicated in patients with a known allergy to isosorbide dinitrate or any of its inactive ingredients. Patients with severe anemia, closed-angle glaucoma, increased intracranial pressure, or concurrent use of phosphodiesterase-5 (PDE5) inhibitors should not use this vasodilator medication.



Possible side effects

The most commonly reported side effect of Isordil is headache, which is often severe at higher doses but typically diminishes with continued use. Lightheadedness related to blood pressure changes is also frequently observed. Hypotension can occur and in some patients may be severe enough to necessitate discontinuation of therapy.

Less common but important side effects include syncope (fainting), crescendo angina (worsening chest pain), and rebound hypertension. This nitrate medication may cause dizziness, palpitations, and weakness due to its blood pressure-lowering effects. To minimize these cardiovascular side effects, patients should rise slowly from sitting or lying positions.

Contact your healthcare provider immediately if you notice bluish discoloration of the lips, fingernails, or palms; irregular heartbeat or palpitations; dangerously low blood pressure; persistent nausea or vomiting; headaches that do not improve; or unusual weakness and fatigue. Less serious side effects that should be reported if persistent include flushing of the face or neck and skin rash.



Drug interactions

The most critically important drug interaction involves Isordil and erectile dysfunction medications. Taking sildenafil (Viagra), tadalafil (Cialis), or vardenafil (Levitra) together with isosorbide dinitrate can dramatically amplify blood pressure-lowering effects and cause life-threatening hypotension, cardiovascular collapse, or death. You must absolutely never combine these medications.

Additionally, do not take Isordil concurrently with other medications for high blood pressure, other nitrate medications for angina, or heart failure drugs without close medical supervision. Alcohol consumption should be strictly avoided, as it potentiates the hypotensive effects of this vasodilator and significantly increases the risk of dangerous blood pressure drops.



Missed dose

If you miss a dose of Isordil, take it as soon as you can. If it is almost time for your next scheduled dose, skip the missed dose and continue with your regular angina treatment schedule. Do not take double or extra doses, as this could cause a dangerous drop in blood pressure.



Overdose

If you suspect an Isordil overdose, contact your local poison control center or seek emergency medical care immediately. Overdose symptoms may include severe headache, persistent throbbing, visual disturbances, extreme dizziness, profound hypotension, rapid weak pulse, nausea, vomiting, and loss of consciousness. Nitrate overdose can be fatal and requires immediate medical intervention.



Storage

Store Isordil at room temperature, approximately 25°C (77°F). Protect from moisture and direct light. Dispense in a light-resistant, tightly sealed container. Keep out of the reach of children and pets. Dispose of any unused medication after the expiration date according to proper pharmaceutical disposal guidelines.
