Common use

Copegus (ribavirin) is a potent antiviral medication used in the treatment of chronic hepatitis C, a serious viral infection that damages the liver over time. Ribavirin belongs to the nucleoside analogue class of antivirals and works by interfering with the hepatitis C virus (HCV) replication process, making it harder for the virus to reproduce within the body.

Copegus is used together with an interferon (such as PegIntron or Intron A) as part of interferon combination therapy to treat chronic hepatitis. This combination approach is prescribed for patients who have not been treated with interferon previously, as well as those who have experienced a return of hepatitis C following previous successful treatment. The ribavirin and interferon combination helps the immune system fight the HCV more effectively, significantly improving the chances of achieving a sustained virological response — meaning the virus becomes undetectable in the blood. Successful HCV treatment can prevent the progression of liver damage, cirrhosis, and liver cancer.

Dosage and direction

The usual dose of Copegus per day is 800 mg, 1,000 mg, or 1,200 mg taken orally in two divided doses (morning and evening) with food for a treatment duration of 24 or 48 weeks. The specific dose and duration of ribavirin treatment depend on your condition, body weight, viral genotype, baseline disease characteristics, response to therapy, and tolerability of the regimen.

Take Copegus exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Follow the directions on your prescription label carefully. Take your antiviral medication consistently at the same time or times each day to maintain steady drug levels in your bloodstream and maximize HCV treatment effectiveness. Always take ribavirin with food, as this significantly improves absorption of the medication.

Precautions

Copegus may decrease the number of red blood cells in your body, leading to anemia. This can be life-threatening in people who have heart disease or circulation problems, as the anemia may aggravate these conditions. Careful consideration and close medical supervision are essential when ribavirin is used in patients with cardiovascular or circulation issues.

Your blood will need to be tested frequently during hepatitis C treatment to monitor for anemia and other blood count changes. Also, your vision, liver function, and thyroid function may need to be tested regularly. Visit your doctor on schedule for all recommended appointments and lab work. Call your doctor at once if you have pale or yellowed skin, dark colored urine, fever, confusion, chest pain, weakness, or trouble breathing — these may be signs of severe anemia or liver problems.

Copegus may cause birth defects and/or death in unborn babies that are exposed to it during pregnancy. If you or your sexual partner are or may be pregnant, do not take Copegus, and contact your doctor immediately. Women and men who are taking ribavirin must use at least two effective forms of birth control to avoid pregnancy during treatment with this antiviral drug and for 6 months after this medication has been stopped. Consult your doctor or pharmacist for more information about pregnancy prevention during HCV treatment.

Contraindications

Before taking Copegus, you should talk with your doctor if you are allergic to peginterferon alfa-2a, any other interferon, ribavirin, polyethylene glycol, or to any of the ingredients of the medication. Do not use Copegus if you are an infant, breastfeeding, pregnant, or have a pregnant partner.

Copegus is contraindicated in patients with: decompensated liver disease (severe liver dysfunction), hemoglobinopathies such as thalassemia or sickle-cell anemia, HIV-HCV co-infection with cirrhosis and a baseline Child-Pugh score greater than or equal to 6, or autoimmune hepatitis.

Discuss with your doctor if you have a severe psychiatric disorder (including depression or suicidal ideation), other autoimmune diseases, or thyroid problems not controlled by medication, even in the past. These conditions require careful evaluation before starting chronic hepatitis treatment with ribavirin.

Possible side effects

The most common side effect of Copegus is a flu-like syndrome consisting of body aches and pains, fever, chills, headache, and malaise. These symptoms are particularly common during the first few weeks of interferon combination therapy and may improve as your body adjusts to the treatment regimen.

The most serious side effect of ribavirin is anemia, which requires regular blood monitoring. Patients with certain types of heart disease should not use Copegus, as severe anemia may worsen cardiac conditions and can lead to a potentially fatal heart attack.

Copegus should be avoided among patients who have severe kidney disease and have lost most of their kidneys' function. Therapy with ribavirin and interferon may aggravate existing psychiatric conditions or may trigger new psychiatric symptoms such as depression, suicidal ideation, psychosis, aggressive behavior, hallucinations, or violent behavior. Patients should be closely monitored for the development of these psychiatric conditions throughout their HCV treatment course.

Therapy has been known to cause abnormalities of the thyroid gland. If these abnormalities persist and cannot be controlled by medication, it may be necessary to discontinue antiviral therapy. Other possible side effects that may occur during treatment with Copegus include fatigue, weakness, nausea, anorexia, heartburn, insomnia, irritability, difficulty breathing, rash, itching, and alterations in taste perception.

Drug interactions

If Copegus is used with other antiviral drugs with similar mechanisms of action, it may cause lactic acidosis, a condition in which lactic acid accumulates in the blood. This situation can lead to serious, potentially life-threatening medical problems and requires immediate medical attention.

Do not take Copegus with certain antiviral drugs used to manage HIV infection, particularly zidovudine (Retrovir), zalcitabine (Hivid), or stavudine (Zerit), as dangerous interactions and increased toxicity may occur. Inform your doctor about all medications you are taking, including over-the-counter drugs and supplements, to prevent harmful drug interactions during your hepatitis C treatment.

Missed dose

If you miss a dose of Copegus, take the missed dose as soon as possible and continue with your regular schedule. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one. Consistent adherence to your ribavirin dosing schedule is important for maximizing the effectiveness of your chronic hepatitis treatment. If you are not sure what to do after missing a dose, contact your doctor or pharmacist for advice.

Overdose

Overdose symptoms of Copegus may include easy bruising or bleeding, urinating less than usual or not at all, chest pain or heavy feeling, pain spreading to the arm or shoulder, nausea, sweating, or a general ill feeling. If you experience any of these symptoms, seek emergency medical attention immediately. Contact your local poison control center for guidance on ribavirin overdose management.

Storage

Store Copegus in the refrigerator, protect from light, and keep out of the reach of children. Do not freeze or shake the medication. Do not use Copegus past the expiry date printed on the packaging. Proper storage of your antiviral medication ensures it maintains its potency and effectiveness throughout your hepatitis C treatment course.
