Common use
Afinitor (everolimus) is a targeted anticancer medication prescribed for the treatment of several advanced malignancies and specific genetic conditions. When you buy Afinitor online, you receive an oral mTOR (mammalian target of rapamycin) kinase inhibitor that works by blocking the PI3K/AKT/mTOR signaling pathway, a critical cellular mechanism that promotes cancer cell growth, division, metabolism, and angiogenesis. Afinitor is approved for the treatment of advanced renal cell carcinoma (kidney cancer) after failure of sunitinib or sorafenib therapy, hormone receptor-positive HER2-negative advanced breast cancer (in combination with exemestane), progressive well-differentiated neuroendocrine tumors (NET) of pancreatic, gastrointestinal, or lung origin, and subependymal giant cell astrocytoma (SEGA) associated with tuberous sclerosis complex (TSC). By inhibiting mTOR, Afinitor disrupts tumor cell proliferation, induces cell cycle arrest, and reduces blood vessel formation within tumors.
Dosage and direction
Afinitor everolimus tablets are taken orally once daily at approximately the same time each day, with or without food, but the chosen method should remain consistent throughout treatment. Swallow tablets whole with a full glass of water; do not crush, break, chew, or dissolve them. The standard dosage for most adult indications is 10 mg once daily, although dose modifications may be necessary based on individual tolerability, adverse effects, and concurrent medications. For SEGA patients, dosing is based on body surface area with target trough levels. Regular monitoring of complete blood counts, serum creatinine, kidney and liver function tests, blood glucose, lipid profile, and urinalysis is essential during therapy. Your oncologist will adjust the dose or temporarily interrupt treatment if significant side effects develop.
Precautions
Before starting Afinitor cancer treatment, inform your oncologist about all existing medical conditions, including liver or kidney impairment, active or chronic infections, diabetes mellitus, hyperlipidemia, a history of interstitial lung disease or pneumonitis, or recent vaccination with live vaccines. Afinitor suppresses the immune system, increasing susceptibility to bacterial, fungal, viral, and opportunistic infections; report any fever, persistent cough, shortness of breath, or unusual symptoms promptly. Avoid contact with individuals who have active infections. Monitor for stomatitis (mouth ulcers and sores), which is among the most common side effects, and use alcohol-free mouthwash as a preventive measure. Avoid grapefruit, grapefruit juice, and Seville oranges during therapy, as they may increase everolimus levels. Women of reproductive potential should use effective contraception during treatment and for 8 weeks after the last dose.
Contraindications
Do not use Afinitor if you have a known hypersensitivity to everolimus, other rapamycin derivatives (sirolimus, temsirolimus), or any component of the tablet formulation. Afinitor is contraindicated during pregnancy due to the risk of fetal harm and should not be used by breastfeeding women. Concurrent administration with strong CYP3A4 inducers that would substantially reduce everolimus exposure should be avoided when possible.
Possible side effect
Common side effects of Afinitor everolimus include stomatitis (mouth sores and ulcers), fatigue, skin rash, diarrhea, nausea, decreased appetite, peripheral edema (swelling), cough, and headache. Metabolic effects such as hyperglycemia, hyperlipidemia, and hypertriglyceridemia are frequently observed and may require medical management. Serious adverse effects requiring immediate attention include non-infectious pneumonitis (interstitial lung disease presenting as cough, dyspnea, and hypoxia), severe infections including opportunistic organisms, hepatotoxicity, renal failure, hemorrhage, and severe hypersensitivity reactions including angioedema. Hematological effects such as anemia, thrombocytopenia, and neutropenia may also occur and require regular blood monitoring.
Drug interaction
Afinitor is metabolized primarily by CYP3A4 and is a substrate of P-glycoprotein (P-gp). Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) can dramatically increase everolimus levels and toxicity, requiring significant dose reductions. Strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine, St. John's Wort) can substantially reduce everolimus effectiveness and should be avoided. Moderate CYP3A4 inhibitors (erythromycin, fluconazole, verapamil, diltiazem) also require dose adjustment. Live vaccines should be avoided during treatment due to immunosuppression. Inform your oncologist about all prescription drugs, over-the-counter products, herbal remedies, and supplements you are taking.
Missed dose
If you miss a dose of Afinitor, take it as soon as you remember if it is within 6 hours of the scheduled time. If more than 6 hours have elapsed, skip the missed dose entirely and take the next dose at the regular scheduled time. Do not take a double dose to compensate for a missed one.
Overdose
Afinitor overdose may cause an intensification of known side effects including severe stomatitis, profound fatigue, increased susceptibility to infections, metabolic derangements, and potential organ dysfunction. There is no specific antidote for everolimus overdose. Seek immediate emergency medical attention if overdose is suspected. Treatment is supportive, and general measures including monitoring of vital signs and organ function should be initiated as clinically appropriate.
Storage
Store Afinitor at controlled room temperature between 20 and 25 degrees Celsius (68-77 degrees Fahrenheit), protected from moisture, excessive heat, and direct light. Keep tablets in the original blister packaging until ready to take and store out of reach of children and pets. Handle with dry hands and do not use after the expiration date.
